สำนักงานความร่วมมือระหว่างประเทศ กรมควบคุมโรค ได้รับข้อมูลในวันที่ 3 ตค 2567 จาก BlueDot เป็นโครงการความร่วมมือของประเทศสมาชิกอาเซียนและประเทศแคนาดาภายใต้ ASEAN Health Cluster 2 ในการแจ้งเตือนเหตุการณ์ของโรคและภัยสุขภาพที่สำคัญ ประกอบกับโรคติดเชื้อไวรัสมาร์บวร์กเป็นโรคติดต่ออันตรายตามประกาศพระราชบัญญัติโรคติดต่อ พ.ศ. ๒๕๕๘ ดังนั้น สำนักงานความร่วมมือระหว่างประเทศ จึงได้ถอดความเป็นภาษาไทย โดยมีรายละเอียดดังนี้

เมื่อวันที่ 2 ตุลาคม 2567 สื่อข่าวหลายสำนักได้รายงานการปิดสถานีรถไฟกลางของเมืองฮัมบูร์กชั่วคราว หลังจากพบผู้ป่วยสองรายแสดงอาการคล้ายไข้หวัดใหญ่ที่อาจเกี่ยวข้องกับการระบาดของโรคติดเชื้อไวรัสมาร์บวร์ก ขณะนี้อยู่ระหว่างการสอบสวนโรค

ข้อมูลปัจจุบัน

• การปิดสถานีรถไฟกลางฮัมบูร์กเกิดขึ้นเป็นเวลาหลายชั่วโมงในช่วงเย็นของวันที่ 2 ตุลาคม 2567 หลังจากที่นักเดินทางสองคนจากเมืองแฟรงก์เฟิร์ตได้เดินทางไปเมืองฮัมบูร์กด้วยรถไฟด่วน Inter-City Express (ICE) และแสดงอาการคลื่นไส้ อาเจียน
• ผู้ป่วยรายหนึ่ง เป็นชายวัย 26 ปี นักศึกษาแพทย์ชาวเยอรมัน ซึ่งมีประวัติการเดินทางไปยังประเทศรวันดา วัตถุประสงค์ของการเดินทางไม่ชัดเจน แต่มีรายงานว่าชายคนดังกล่าวได้สัมผัสกับผู้ป่วยที่ภายหลังถูกวินิจฉัยว่าเป็นโรคติดเชื้อไวรัสมาร์บวร์กที่ประเทศรวันดา สำหรับข้อมูลเกี่ยวกับผู้ป่วยอีกหนึ่งราย ยังไม่ชัดเจน
• ยังไม่ทราบแน่ชัดว่า ผู้ป่วยทั้งสองคนถูกตรวจพบได้อย่างไร แต่มีรายงานว่า ทั้งสองคนถูกแยกกักตัวที่ชานชาลาของสถานีในฮัมบูร์ก และถูกย้ายไปยังหน่วยแยกโรคที่โรงพยาบาลมหาวิทยาลัย Eppendorf โดยรถพยาบาลกู้ชีพโรคติดเชื้อของหน่วยดับเพลิงฮัมบูร์ก และได้มีการเก็บตัวอย่างตรวจทางห้องปฏิบัติการเพื่อหาสาเหตุของการเกิดโรค
• ในขณะเกิดเหตุมีผู้โดยสารประมาณ 200 คนบนรถไฟ ICE และทางการเยอรมันได้เริ่มติดตามอาการผู้สัมผัสแล้ว

ข้อมูลเพิ่มเติม

• ประเทศรวันดามีการรายงานการระบาดของโรคโรคติดเชื้อไวรัสมาร์บวร์กภายในประเทศเมื่อวันที่ 27 ก.ย. 2567 โดยเฉพาะอย่างยิ่งพบในกลุ่มบุคลากรทางการแพทย์ ซึ่งข้อมูลการป่วยในรวันดา ณ วันที่ 1 ต.ค. 2567 มีรายงานผู้ป่วย 29 คน และผู้เสียชีวิต 11 คน
• จากข้อมูลการเดินทางทางอากาศระหว่างประเทศรวันดาและประเทศเยอรมันนีในช่วงเดือนกันยายนและตุลาคมนั้น  พบว่า จำนวนนักเดินทางของประเทศเยอรมันนียังไม่อยู่ใน 10 อันดับแรกของจำนวนนักเดินทางจากประเทศต่างๆ ที่มีการเดินทางไประหว่างกันกับประเทศรวันดา

ระดับความกังวล

• ขณะนี้มีข้อมูลที่ไม่ชัดเจนหลายอย่างเกี่ยวกับเหตุการณ์นี้ รวมถึงวันที่และสนามบินที่ผู้ป่วยสงสัย 2 คน เดินทางมาถึงเยอรมนี รวมถึงประวัติสัมผัสต่างๆ และการสัมผัสใกล้ชิดกับบุคคลอื่นๆ
• เชื้อไวรัสมาร์บวร์กสามารถติดต่อผ่านการสัมผัสกับสารคัดหลั่งจากร่างกายผู้ติดเชื้อ จึงมีความเสี่ยงในการแพร่ระบาดในพื้นที่ที่มีประชากรหนาแน่น เช่น สนามบินและสถานีรถไฟ รวมถึงยานพาหนะสาธารณะเช่น เครื่องบิน รถไฟ และรถโดยสาร
• การตรวจพบเร็ว การติดตามผู้สัมผัสอย่างรวดเร็ว และมาตรการควบคุมการติดเชื้อที่เข้มงวดเป็นสิ่งสำคัญในการป้องกันการแพร่ระบาด ซึ่งทางหน่วยงานสาธารณสุขในฮัมบูร์กได้เริ่มดำเนินการแล้ว

Marburg Virus Disease in Germany

 EVENT DESCRIPTION
 
On 02-Oct-2024, multiple news media outlets reported a temporary closure of the Hamburg Central Station after two individuals developed flu-like symptoms, based on case details Marburg virus disease (MVD) is under investigation. This situation is still developing, and more details are expected soon.

Current Information:

The Hamburg Central Station closure occurred for several hours during the evening of 02-Oct-2024, after a couple traveling from Frankfurt to Hamburg on the Inter-City Express (ICE) train developed symptoms such as nausea and vomiting.
Of concern, one affected individual is described as a 26-year-old male German medical student, who had recent travel history to Rwanda. The purpose of their travel is not described but it is reported that the individual had contact with a patient who later was diagnosed with MVD. Details about his companion remain unclear.
It is unclear how their condition was flagged to authorities, however, it is reported that they were initially isolated on the platform in Hamburg and were moved via the Hamburg Fire Department's infectious disease rescue vehicle to an infectious disease isolation unit at the University Hospital Eppendorf.
Laboratory testing is currently underway to determine the source of their symptoms.
Around 200 passengers were on board the ICE train at the time, and contact tracing efforts have begun.

Additional Information:

Rwanda reported a MVD outbreak within the country on 27-Sep-2024, particularly affecting health care workers. As of 01-Oct-2024, 29 human cases and 11 deaths have been reported in Rwanda.
Air travel data indicates that Germany is not among the top 10 countries for forecasted passenger volume to Rwanda in September or from the country in October.
 
CONCERN LEVEL

There are many unknowns regarding this event at this time including the couple's arrival date and airport in Germany, any other notable interactions they may have had, and what potential additional sources of infection are under investigation.
MVD's potential for transmission through contact with infected bodily fluids poses a heightened risk of transmission in densely populated areas such as airports and train stations and their accompanying means of transportation like planes, trains, and buses.
Early detection, rapid contact tracing, and strict infection control measures are crucial for preventing spread. The appropriate actions have been initialized by health authorities in Hamburg.
